(P 268) QUALITY OF LIFE OUTCOMES IN PEDIATRIC PATIENTS UNDERGOING SURGICAL INTERVENTION FOR LOWER EXTREMITY BONE SARCOMAS

Objective: Multiple surgical options are available for pediatric patients with lower extremity bone sarcomas, though the influence of these interventions on quality of life (QoL) outcomes is not well described. This study examines differences in QoL based on surgical intervention over time.

Methods: This is a prospective cohort study performed at a single institution. SF-36 health surveys were administered pre-operatively and throughout long-term follow up. The relationships between surgical intervention (rotationplasty, amputation, intercalary allograft, endoprosthetic reconstruction), reoperation, and QoL scores were assessed using mixed linear models and ANOVA tests.

Results: 84 patients met inclusion criteria. SF-36 General Health scores of patients with endoprosthetic reconstruction had a yearly average decrease of 1.18 (PEMT=0.036, 95% CL = [-2.27, -0.079]) controlling for the main effects of number of reoperations (PANOVA=0.26). SF-36 Pain scores for patients with intercalary allograft reconstruction had a linear relationship with time showing a decrease of 3.709 points per year (PEMT=0.0008, 95% CL = [-5.86, -1.56]), while controlling for the main effects of the number of reoperations (PANOVA=0.014). When all patient reported outcome time points are averaged, there is no significant difference between tumor location or surgical intervention after pairwise analysis (p>0.05).

Conclusion: Differences in the long-term patient reported QoL scores are multifactorial but seem to be related to the need for revision surgery. These findings may help counsel patients regarding expected outcomes. Limitations include survey compliance, loss to follow-up, and sample size.
