(P 469) THE IMPACT OF PREOPERATIVE ANEMIA ON POSTOPERATIVE COMPLICATIONS AND SHORT-TERM PROGNOSIS IN RETROPERITONEAL TUMOR SURGERY PATIENTS.

Objective: Investigate preoperative anemia's effect on postoperative complications and short-term prognosis in retroperitoneal tumor surgery patients, and pinpoint risk factors for severe complications.

Methods: A retrospective cohort study included 1,968 patients undergoing retroperitoneal tumor surgery at Peking University International Hospital (2015-2023). Patients were divided into anemia and non-anemia groups based on WHO hemoglobin criteria. Preoperative data were collected. Primary outcome: postoperative complication severity (Clavien-Dindo classification). Secondary outcomes: ICU stay, mechanical ventilation duration, hospital stays, and 24-hour/30-day mortality. Statistical analyses were conducted using R software.

Results: Among 1,968 patients (46.4% males, 53.6% females), 55.9% had preoperative anemia. Baseline characteristics differed significantly between groups (P < 0.001). The anemia group had a higher rate of severe complications (10.8% vs. 5.8%, P < 0.001), longer total (median: 31 vs. 26 days, P < 0.001) and postoperative (18 vs. 15 days, P < 0.001) hospital stays, extended mechanical ventilation (P < 0.001) and ICU stays (P < 0.001), and a higher 30-day mortality rate (1.5% vs. 0.5%, P = 0.029). Multivariate analysis identified sex (OR = 0.6, P = 0.003), age (OR = 1.014, P = 0.025), and preoperative anemia (OR = 1.9, P < 0.001) as independent risk factors for increased complication severity.

Conclusion: Preoperative anemia, sex, and age are independent risk factors for severe postoperative complications in retroperitoneal tumor surgery. Anemia is linked to longer hospital stays, extended ventilation, and increased ICU stays.
