(P 124) ISOLATED LIMB PERFUSION AS LIMB SALVAGING TREATMENT; A SINGLE CENTRE RETROSPECTIVE STUDY

Objective: Isolated limb perfusion (ILP) is a limb-sparing treatment modality for patients with locally advanced extremity sarcomas. This retrospective study evaluates the clinical response rates to ILP over a 20-year period.

Methods: We reviewed a prospectively maintained database of Swedish patients who underwent ILP due to locally advanced extremity sarcomas. Clinical responses were categorized as complete response (CR), partial response (PR), stable disease (SD), and progressive disease (PD) based on post-treatment evaluations at three months.

Results: Between 2004 and 2024, 92 Swedish patients with locally advanced extremity sarcomas were identified. 14 patients were excluded (adjuvant treatments, non-perfused patients). A total of 78 patients (female n=37, male n=41) were response-rated, with a mean age of 59 years. All patients received Melphalan+TNF-alfa except one who was given only Melphalan. At follow up CR was observed in 17% (n=13), PR in 45% (n=35), SD in 28% (n=22), and PD in 10% (n=8).

Conclusion: ILP for locally advanced extremity sarcomas demonstrates high efficacy, with 62% overall response rate resulting in delayed or spared amputation. These findings support the continued use of ILP as a valuable therapeutic strategy in limb-sparing management of sarcoma.
